The Parent's Bill of Rights (F.S. 1014.06(1)) requires that we receive parent/guardian consent for delivering any clinical services to students on our campus. In order to treat a student in our school's clinic, we need a consent form on file indicating that we have your permission.
A “yes” response on the Healthcare Consent form will authorize such treatments including, but not limited to, major or minor injury or illness reported or observed while the child is at school. This does not authorize the dispensing of medication or school screenings such as vision, hearing, scoliosis, or height and weight. These services require a separate consent which is included in the original registration paperwork.
If a "no" response is indicated on the Healthcare Consent form, the student will not be treated in our school clinic and their parent/guardian will be called for all (minor or major) medical concerns. Students who do not have a completed Health Care Consent form on file with the school will be treated as if the parent/guardian selected "No" for all clinic services.
Medications will continue to be dispensed as usual, with the appropriate documentation on file.
We will continue to triage all students in emergency situations and call for paramedics as necessary, even if we have a Health Care Consent form on file.
